RISK MANAGEMENT PROCEDURE

• Go to http://www.enysoccer.com

• Use the Risk Management Tab

• Certified Referees, Coaches, and Volunteers Requiring Background Check. Please select continue.

• For Online Volunteer Disclosure Form, select "Mid-State (MSNYL).

• Log in and follow the instructions.

When you finish, you should be able to "View Order" and print a copy.  You should also receive an e-mail confirmation.  Forward the e-mail attachment to Rick Scholl, for registration, with a copy to Sherry Heenan for reimbursement.
